A funeral service will be at 10:00 AM on Saturday at Chapel Hill Baptist Church, 1521 Boyd Rd, Utica, MS, with Bro. Walter Forman officiating. Mr. Forman is survived by his parents: Mark and Patricia Forman of Utica; stepsister Stephanie Forman of Houston, TX; grandparents: Walter and Beatrice Forman of Gloster, MS and a host of aunts, uncles and family members.
He was preceded in death by grandparents: Clarence and Betty Hill.
